We are looking for an individual with a professional and ethical approach to their work to join our Health and Local Crisis Response Team as a Healthcare Assistant in Nottingham. A day in the life of a Healthcare Assistant will involve:
- Providing invaluable contribution to the Community Crisis Support Service and its users.
- Providing short term crisis support to people in their homes, helping to keep them out of hospital.
- Assessing clients' needs and develop a care plan to fit them.
- Providing support with personal care, medication prompts, end of life care, light meal and drinks preparation and emotional support.

To be a successful Healthcare Assistant, You’ll need:
- NVQ Level 2 or equivalent in health and social care
- Ability to communicate with health and social care professionals
- Ability to deal with service user queries in a diplomatic, professional and confidential manner
- Knowledge of supporting service users, who may be disabled and/or frail, cognitively impaired.
- Good geographical knowledge of the service catchment area.
- IT literate
- Knowledge of services provided by the NHS and Social Care
- Good working experience of the delivery of social care support including personal care, feeding and medication prompting
- Participation in a multi-disciplinary team environment
- Full driving licence holder and access to vehicle, ability to use the vehicle daily for service user visits
- Willingness to work flexible hours
