Salary: £22,932 (plus Inner London Weighting £3,344 if applicable) per annum pro rata

What will a day in the life of a Support Worker involve?
- Providing practical and emotional assistance to people returning from hospital or medical facilities.
- Accompanying service users to attend hospital appointments and other activities.
- Liaising with health, housing and other professionals to ensure that the needs of the service user are being consistently met.
- Ensuring a safe living environment for service users by undertaking risk assessments.
- Carrying out welfare checks to assist service users with day-today activities.
- Building positive relationships with service users, their families and other healthcare professionals.
- Ensuring all care documentation is accurate and up to date.
What does it take to be a successful Support Worker?
- Previous experience in a similar care, preferably within a healthcare or social care setting.
- Compassionate and empathetic nature, with a strong desire to make a positive impact on individuals’ lives.
- Excellent communication and interpersonal skills, with the ability to work effectively within a multidisciplinary team.
- Knowledge of person-centred care principles and a commitment to upholding dignity and autonomy.
- Ability to work independently, demonstrating initiative and reliability.
- Flexibility to work various shifts, including evenings, weekends, and bank holidays.
- A full driving license and access to a vehicle is preferred but not mandatory.
