About The Role

Support Worker – Assisted Discharge Service - Gloucestershire

Location: Gloucestershire Royal Hospital, GL1 3NN
Salary: £22,932 per annum (pro-rata for under 35 hours) Job Share Available
Hours: Various hours available. Shifts Available 10am to 8pm weekday and 10am to 5pm weekends (working one weekend per month) 
Contract: Permanent
Driving Requirement: Full UK Driving Licence held for a minimum for 2 years

What happens when someone is ready to leave hospital but has no one to help them get home or settle in? That’s where you come in.

As a Support Worker in our Assisted Discharge Service, you’ll help vulnerable adults return home safely and comfortably. You’ll assess their needs, ensure their home is ready, and provide short-term support to reduce the risk of readmission.

What will a day in the life of an Assisted Discharge Worker look like?

  • Transporting patients’ home and ensuring their environment is safe and comfortable
  • Collecting prescriptions, checking food supplies, and completing light household tasks
  • Offering emotional support and guidance, helping service users regain independence
  • Liaising with hospital staff and responding to referrals from clinicians

Note: This role does not involve personal care (e.g. bathing, cleaning, or administering medication).

What will you need to be a successful Assisted Discharge Worker? 

  • The Right to Work in the UK (we’re unable to offer sponsorship)
  • A full UK manual driving licence and access to a vehicle
  • Understanding of NHS and Social Care services
  • Professionalism, discretion, and flexibility to work outside standard hours
